I certify and attest that I have the authority to submit a campaign registration for the company above. I also acknowledge that I have verified all of the data prior to submission and remain responsible for any errant data and/or the outcome of this submission. I further understand and acknowledge that by submitting a campaign registration request that there will be a one-time charge of $20 added to our next invoice, and that this charge is non-refundable and that each campaign use case has an associated monthly cost, along with carrier imposed fees, surcharges, fines, costs.
